The Patient Facilitator aides the Primary Care team with patient flow through a variety of tasks.
The Patient Facilitator will ensure exam rooms are stocked and cleaned.
The Patient Facilitator will retrieve the patients from the waiting room and escort them to the triage or exam room.
This person always maintains professionalism communication with the patient and delivers care in a culturally competent manner and in the patient's native language.

The Patient Facilitator will ensure exam rooms are stocked and cleaned.
The Patient Facilitator will retrieve the patients from the waiting room and escort them to the triage or exam room.
This person always maintains professionalism communication with the patient and delivers care in a culturally competent manner and in the patient's native language.
This position requires compliance with the Center's Compliance Program and Standards of Conduct and applicable policies and procedures.
Such compliance will be considered as part of a regular performance appraisal.
Failure to comply with the Center's standards, including the failure to report any conduct or event that potentially violates legal or compliance requirements, will be met by the enforcement of disciplinary action, up to and including possible termination.
Essential Functions:
Prepares exam rooms for patient treatment as per GPWCHC protocols.
Maintains/cleans all exam rooms in compliance with GPWCHC protocols and procedures relative to infection control, exposure control and safety issues.
Prepares patients for the appointment by bringing them to the triage or exam room and taking vitals.
Effectively communicates with the clinical team members.
Maintains adequate medical supplies and compiles a list of individual item shortages for inventory control and ordering purposes as assigned.
As directed by a supervisor, performs other related and/or necessary tasks as assigned.
Qualifications, Education, and
Experience:
High School Diploma Bilingual (English/Spanish) a Plus Excellent communication skills Highly organized and motivated to be a valuable team member.
